Soft-baked bars for toddlers recalled due to choking hazard

The Hawaii Department of Health is alerting residents to a recall of certain Once Upon A Farm snack bars over a potential choking hazard.

The U.S. Food and Drug Administration is advising consumers not to buy or serve Once Upon A Farm Tractor Wheels, Organic Toddler Soft-Baked Bars, including all flavors, quantities, and varieties, and all lot codes with expiration dates of Oct. 30, 2026 to Aug. 26, 2027.

“The FDA has received several reports that describe toddlers (ages 1.5–2.5 years) suffering complete airway obstruction with the inability to breathe after consuming the product,” said the FDA in its notice. “When chewed, the product reportedly forms a dry, sticky mass that standard choking interventions — including back blows and the Heimlich maneuver — may fail to dislodge.”

The FDA added, “Some reported choking incidents have required emergency response intervention and hospital observation.”

The recalled products were distributed nationwide at retailers such as Walmart, Target, Costco and other stores in Hawaii — and were also available for purchase online and on Amazon.com.

Once Upon a Farm of Berkeley, Calif., issued the voluntary recall on Oct. 31 after three consumer complaints were reported to the FDA.

The company said no injuries have been reported to date, and that it was working with the FDA to make sure all affected products are removed, and families have the information they need.

”As part of this process and a larger evaluation of Tractor Wheels products, we are reformulating these family favorites to ensure they adhere to the highest standards,” said co-founder and CEO John Foraker in a written message.

The recall includes all lot codes currently available for sale, as well as expired products which may still be on consumers’ shelves.
